If you had Obamacare health insurance or insurance through the health insurance marketplace program, you are required to file this form with your tax return.
The form is called 1095-A and is mailed to recipients of this insurance at the beginning of the following year. If you don’t receive this form in the mail, it is important that you get it and include it on your personal tax so you don’t file late or send your file missing important documents.
Here is how to get it if you didn’t receive your form by mail.
Log on to your online account
If you signed yourself up for health insurance through the marketplace, you can get your form by logging on to the healthcare.gov site into your account.
Once you are in the account, navigate to where it shows the notices you have been issued. Under this page, there should be one that says your 1095-A form is ready for viewing.
There, you can download the form to bring with you when you go to file your taxes.
Contact your agent
If you were signed up for your health insurance by a health insurance agent, you can call this agent to request your form.
The agents have the form available and can send it immediately upon request. If you call, email, or text your agent they can send a copy of the form digitally for you to bring when you file your tax return.
Call the health insurance marketplace
If none of the above methods work for finding your form, you can also find your form by calling directly to the marketplace and speaking to a marketplace representative.
The marketplace is able to send another copy to you via post mail, and they are also able to read the information from your form to you on the phone.
If you go to their site and print out a blank 1095-A form, you can use that to fill in the information the marketplace associate reads to you on the phone and bring that paper to your tax preparer to file your taxes while you wait to receive the copy in the mail.
